Dear Recipients,
As part of a grant funded by the Centers for Disease Control and Prevention (CDC), the Montana Department of Public Health and Human Services is announcing two Diabetes Program funding opportunities to work on projects related to accessing and addressing social determinants of health (SDOH). Project one is focused on screening for SDOH and project 2 is focused on providing training on assessing and addressing SDOH among priority populations with diabetes.
