Did you or someone in your household recently lose MO HealthNet coverage? Take action soon to get your coverage back! If you fill out and return your annual renewal form within 90 days of losing coverage we may be able to reinstate your coverage without any lapse. If you can't find your form we can still help you! Complete your annual renewal by creating an account and completing your form in the FSD Benefit Portal, starting a chat with us online, or calling 855-373-4636.
If you do not know when your annual renewal is due, it is typically required around the anniversary of when your coverage began. If you have not yet received an annual renewal form or just want to check when it is due you can create an account in the FSD Benefit Portal.
