DHSS UPDATES

► PHIG Monitoring Assessment: Reporting Period 6 (RP6) covers the time period from June 1st 2025 through November 30th, 2025. Your PHIG Monitoring Assessment report for RP6 is due by January 15, 2026 via Redcap. Information from the PHIG reports you submit help DHSS make the case for continued funding. If you have exhausted your agency’s PHIG funds, please continue to complete the remaining PHIG Monitoring Assessments through the end of the PHIG funding period ending in November, 2027. It is important to evaluate as much of the LPHA workforce as possible during the next two years to make an argument for continued funding beyond 2027 and to report required data to the CDC. Contact Travis Fisher with any questions or for assistance.

Migration of all records, case notes, attachments, etc., was completed in October. The vendor is still working on a few changes to screens on various conditions, but these changes should be relatively minor and should not prevent end users from working cases. Contact information is listed below and will be added to the LPHA Info Hub.
