National Family Caregivers Month, is recognized in November of each year. Special Health Care Needs Family Partners invite you to honor the caregivers you know by nominating an unpaid family member or friend for the Missouri Caregiver of the Year. Caregivers are often the biggest cheerleader for your loved one but are not always recognized for the role they play in providing informal care to children and youth who have been identified as having complex medical condition(s) and/or who are deaf and hard of hearing.
The department will feature a story and photo of an unpaid caregiver each month. By December, an overall Caregiver of the Year will be selected. If you would like to nominate someone, complete the nomination form and the photo release form. Send both forms, along with a digital photo of the caregiver, to dsdsofficeofconstituentservices@health.mo.gov.
