The New Power of Attorney Form REV184

Based largely on feedback from customers, we have simplified our Power of Attorney (POA) Form REV184. To simplify the form, we:
- Clarified and simplified language within the form and instructions.
- Added space for taxpayers to clearly list their primary appointee.
- Reorganized and streamlined the instructions to provide clear step-by-step guidance.
- Added the ability to appoint multiple representatives by including an attachment.
We also added optional elections to reduce paperwork for you and your clients. Before this change, taxpayers had to fill out up to three forms to appoint a POA, depending on what authority they were granting. Now they can use a single form to do any or all of these things:
- Appoint you as their POA.
- Authorize us to send all of their correspondence, including refunds, to you rather than to the taxpayer.
- Authorize you to communicate with us by email about their tax matters.
In addition, please note that:
- To enhance the security of taxpayer information, we have removed the ability to appoint a firm (rather than individuals) as POA on this form.
- We will no longer accept the previous version of Form REV184 after December 31, 2014.
- We have eliminated Form REV184a, Election for Power of Attorney To have Appointee Receive All Correspondence from the Department of Revenue.
A diverse group of customers participated in usability testing of the old and new forms including individual taxpayers, military personnel, business owners, attorneys, and tax preparers. Many of our changes came from their feedback. Thank you for helping us improve and simplify this form.
