Get reimbursed for over-the-counter COVID-19 tests
The Minnesota Advantage Health Plan now covers over-the-counter COVID-19 tests free of charge. This is effective for tests purchased on or after January 15, 2022.
For each person covered under your plan, there’s a limit of eight (8) tests per 30-day period. Tests remain in short supply so your pharmacy may limit the number of tests you can purchase in one visit. Reimbursement is limited to $12 per test.
To ensure the cost of your test is covered, there are two options:
1. No payment required at in-network pharmacy: Present your CVS prescription benefit ID card to any network pharmacy that elects to offer COVID-19 tests at no out-of-pocket cost to you. To find participating pharmacies, sign in to caremark.com and use the pharmacy locator.
2. Purchase a test from any local retailer or online, then submit a request for reimbursement: If you do not have your CVS prescription benefit ID card or you purchase the test at a location that does not participate in the program, you can submit a claim for reimbursement to CVS Caremark.
To submit a request for COVID-19 test reimbursement:
- Save your receipt as proof of purchase.
- Sign in to caremark.com or the CVS Caremark Mobile App and submit your request. If you don’t have an account, you will need to register for an account online.
- Reimbursements must be made through CVS Caremark and NOT your medical plan administrator.
