Sign up online. You can also sign up online using the COVID-19 Vaccine Connector. Shortly after you register, you will be contacted to schedule an appointment.

Do not arrive more than 15 minutes early. To help maintain proper social distancing, please do not arrive for your appointment more than 15 minutes ahead of schedule.

Bring your insurance card if you have one. The vaccine is provided at no cost, however please bring your insurance card if you have one. This allows the state to bill your insurance company on your behalf. If you are uninsured or your insurance does not cover some or all of the costs, the state will cover the difference.

Plan to wait for 15 minutes after your vaccine. After you receive your vaccine, we will observe you for at least 15 inutes to ensure there is no adverse side effects. If you have allergies, you will need to wait for at least 30 minutes.

Requirements for check-in. You must wear a mask throughout your entire visit. At the time of check-in you will need to confirm your name, date of birth, and home address.
