IRS Form 1095-C to hit the mail soon

Form 1095-C includes information about the health insurance coverage offered to you by the State of Minnesota.

If you consented to receive this form electronically before January 23, 2019, it is now available in Self Service.

If you did not consent, look for the form in your home mailbox in mid-February.

We encourage you to consent to receive your future 1095-Cs electronically. Just go to Self Service, select Benefits, and then 1095-C Consent.

You do not need your 1095-C to file your taxes. As your employer, the state will provide a copy of your 1095-C to the IRS.

Please review your 1095-C and if you have questions contact SEGIP at 651-355-0100.

Begin the year with a benefit review

Make sure your benefits are in place for 2019 by making a few quick reviews.

Review the medical cards for yourself and your dependents

  • Check the Primary Care Clinic listed on each card. If it is not the clinic you want, contact the customer service number listed on the back of the card.
  • Make sure the cost level of your Primary Care Clinic is correct. If you think it is wrong, check the cost level using the clinic directory on the SEGIP website. If you believe your card is incorrect, contact the customer service number listed on the back of your card.
  • Make sure that your $5 copay discount has been applied if you completed your health assessment during Open Enrollment. Contact Virgin Pulse Member Services if you believe it has not been applied and should have been.

Need a replacement card?

If you have lost or misplaced a card, please call the vendor. Visit the Insurance Contacts page of the SEGIP website for their contact information.

Review your benefits summary in Self Service

  • Review your covered dependents to make sure any enrollments or disenrollments you made during Open Enrollment were applied. Contact SEGIP if they are not listed.
  • Take a look at the pre-tax elections you made. Did you sign up for a Dependent Care Expense Account but you have no children? Parking Expense Account but you don’t own a car? Contact SEGIP as soon as possible if you signed up for an incorrect pre-tax account.
  • Make sure that you are not carrying Spouse Life, Spouse Accidental Death and Dismemberment, or Child Life insurances for dependents who are not eligible for coverage. For example, if your last child is over age 26, you need to drop Child Life and if you are not married, you do not need spouse life.

Review your paycheck deductions

  • Compare your benefits summary with your paycheck to make sure that your insurance deductions are correct and contact SEGIP immediately if they are not.

Did you change your medical carrier during Open Enrollment?

You will need to take action if you participate in these programs:

  • Advantage Value for Diabetes (AVD): Contact CVS Caremark to let them know you changed carriers if you, or a covered family member, are participating in AVD. CVS Caremark will reset the AVD benefits so your prescription drug benefit under AVD continues.
  • Point of Service (POS): You must complete a new POS form and submit it to SEGIP so your POS benefits stays in place if you, or a covered family member, had this program in place.
