Veteran Health Navigator Program
Better care for Veterans starts with knowing who they are.
The Minnesota Department of Veterans Affairs (MDVA) Veteran Health Navigator (VHN) Program helps healthcare organizations identify Veterans and understand their military service history. The program connects Veterans to the VA and community resources while strengthening care coordination, behavioral health services, and suicide prevention efforts.
MDVA promotes several initiatives through the VHN Program:
-
Identify Veterans - You can't support a population you can't see
-
Connect Veterans to Care - The right resources, at the right time
-
Advance Population Health - Turn identification into meaningful action
-
Become a Veteran-Ready Healthcare Partner - Make a difference now
-
Strengthen Behavioral Health & Suicide Prevention Efforts - Build on the systems you have in place
Get started by visiting Minnesota Department of Veterans Affairs or emailing vhn.mdva@state.mn.us
