Sustainability Summit Session 3: Health Care Homes as a Sustainability Lever

Join us for the final session of the Health Care Homes Sustainability Summit 2026 series, October 1, 8 – 9 a.m.

This panel discussion brings together leaders in rural health, population health, and pediatric complex care to explore how the HCH model strengthens sustainability through participation in networks and ACOs, community partnerships, and integrated team-based care. Panelists will share practical, real-world approaches for aligning clinical and financial strategy while improving patient outcomes and experience.

CME Credit: This activity has been planned and implemented in accordance with the accreditation requirements of the Accreditation Council for Continuing Medical Education (ACCME), through the joint providership of the Minnesota Medical Association and MDH Health Care Homes. This session offers 1 AMA PRA Category 1 Credit™; physicians should claim credit commensurate with the extent of their participation.

Did You Know? Health Care Homes Offers Arthritis Management Training

Health Care Homes, in partnership with the MDH Office of Statewide Health Improvement Initiatives, offers two e-learning courses to help care teams better support patients living with arthritis. Clinic-Community Linkage for Arthritis Management walks learners through the screen, counsel, referral, and follow-up framework — including use of the 5 A's and the Walk With Ease program — while Supporting Arthritis Management: The Role of Community Health Workers explores how community health workers can extend that same support directly to patients.

Together, these courses offer practical tools for identifying arthritis risk, connecting patients to evidence-based physical activity programs, and communicating effectively about management options.

These courses are available exclusively to Health Care Homes certified clinics. If you don't have access information for the courses, please contact your Practice Improvement Specialist. If you are unsure who your specialist is, email health.healthcarehomes@state.mn.us.
