Join us Wednesday, September 24, 8:00–9:00 a.m. for Session 2 of the Health Care Homes Sustainability Summit: The Sustainability Equation: Blended Revenue, Value, and Impact. This panel dives into reimbursement models, cost savings, alternative payment strategies, and the health plan perspective on sustaining high-value primary care.

Health Care Homes E-Learning Spotlight: Integrating Primary Care and Behavioral Health

Explore practical approaches for bringing primary care and behavioral health services together with our e-learning course, Integrating Primary Care and Behavioral Health: Approaches and Strategies.

Created through a collaboration between two Minnesota Department of Health sections—Health Care Homes and the Office of Statewide Health Improvement Initiatives—this course highlights best practices in integrated care and Minnesota-based examples from primary care and behavioral health providers.

In this course, you’ll learn how to:

  • Define bidirectional integration and explain the need for this approach

  • Identify three key components of integrated care and how they can be implemented in primary care or behavioral health settings

  • Describe the experiences, challenges and solutions of Hennepin County Mental Health Center in developing its integrated care delivery model

Learn from real-world experiences and gain strategies to support integrated, whole-person care in your own setting.
