Save the Dates: Health Care Homes Sustainability Summit 2026

Join us for the Health Care Homes Sustainability Summit 2026, a free virtual educational webinar series taking place on September 17, September 24, and October 1 from 8:00–9:00 a.m. each day.

Designed for healthcare leaders, clinicians, administrators, finance and population health professionals, payers, and policymakers, this three-part series offers practical education focused on sustaining high-quality, patient-centered primary care.

Throughout the series, participants will learn from national and regional experts as they explore topics including:

  • Workforce innovation and community-based care
  • Blended revenue and value-based care strategies
  • Leveraging Minnesota's Health Care Homes within healthcare networks and accountable care organizations (ACOs)

Each session includes expert presentations, panel discussions, and practical strategies that participants can apply within their own organizations to build resilient, sustainable systems of care.
