Building Sustainable Primary Care: Health Care Homes Sustainability Summit 2026

Join us for the Health Care Homes Sustainability Summit 2026, a free virtual webinar series featuring three one-hour sessions on September 17, September 24, and October 1, 2026, at 8:00 a.m.

This series will explore strategies for sustaining high-quality, high-value primary care through: • Workforce innovation and community care • Blended revenue and value-based care strategies • Leveraging Minnesota's Health Care Homes within networks and ACOs
