Greetings! 

This winter and spring have brought a number of transitions to Health Care Homes. We’ve seen both three incredibly valued long-time staff members retire and the arrival of a new and amazing individual to our team! You’ll hear a little more on this later in the newsletter, but I’d be remiss if I didn’t take a moment to express my gratitude towards the former and welcome to the latter. 

As always, things continue marching along at their usual busy pace. There have been multiple clinic recertifications and ongoing provision of technical assistance and support. The learning team has had plenty to do, with multiple webinars, work on an e-learning course, and, of course, the upcoming Learning Days Conference. I’m particularly excited about this year’s conference! With a keynote, 16 workshop sessions, an exhibit hall, and plenty of opportunities for networking, it looks to be another great year of learning and connection. I also want to take a moment to acknowledge the fact that our strategic planning process is speeding towards a final conclusion. Things are due to wrap up in June, but we will begin sharing some of the results in the not-too-distant future. In any case, thanks as always to everyone for the important work being done to protect the health of Minnesotans! It’s a privilege to work with all of you.

Learning Update

Planning for Learning Days 2026 is well underway, and we’re pleased to share that preparations are progressing well. Our schedule is officially set, and we are looking forward to delivering a high-quality, engaging experience tailored for clinical professionals across the region.

Join us on May 13, at the Heritage Center of Brooklyn Center in Brooklyn Center for a day focused on learning, connection, and practical insights you can bring back to your organization.

This issue of the HCH Connection marks the retirement of three valued members of our program whose dedication, expertise, and leadership have helped shape Health Care Homes for many years. As they begin this next chapter, we want to thank them for their outstanding service, collaboration, and friendship. Their contributions have left a lasting impact on our program and the many colleagues, clinics, and communities they have supported.
