This essential tool is an evidenced-based lifestyle change program aimed at helping participants make lasting changes like increasing physical activity, making healthy food choices, managing stress, and more! The program is covered by most health plans, including Medicare, and can be delivered to groups in-person or online by health care systems and community-based organizations.
