Webinar: The Lifestyle Change Program: Evidence-Based Support for Improved Patient Health

This webinar is an opportunity for providers, care team members, and clinic leaders to learn more about the Lifestyle Change Program. This essential tool is an evidenced-based lifestyle change program aimed at helping participants make lasting changes like increasing physical activity, making healthy food choices, managing stress, and more! The program is covered by most health plans, including Medicare, and can be delivered to groups in-person or online by health care systems and community-based organizations. Learn how MDH staff can support you as you explore offering the Lifestyle Change Program or referring your patients to an existing program.

Quit Partner is Minnesota’s free way to quit nicotine, including smoking, vaping and chewing. The Health Systems Change team at National Jewish Health will be giving a presentation on the family of programs available through Quit Partner. This will highlight the various programs and services available, including information on the different referral pathways.
