Save the date for Learning Days

Two Days Left to Submit a Proposal for Health Care Homes Learning Days!

There are two days remaining to submit a presenter proposal for the upcoming Health Care Homes Learning Days event.

Health Care Homes Learning Days offers an opportunity for providers, care coordinators, administrators, and partners to share practical strategies, lessons learned, and innovative approaches that strengthen patient-centered, community-based care across Minnesota.

We encourage individuals and organizations who are considering submitting a proposal—or who may have been waiting to finalize details—to take advantage of the remaining time. Submissions that reflect real-world experience, quality improvement efforts, and collaborative work within the Health Care Homes model are welcome.

The deadline to submit a proposal is Friday, February 13 at 5:00 p.m.

Webinar: Minnesota’s Action Plan to Address Cardiovascular Disease, Stroke, and Diabetes 2035

Participants will explore the Plan’s key goals and priority outcomes, learn how it was shaped through community engagement, and identify practical opportunities for partnership and collaboration. Attendees will leave with a clearer understanding of how the MN 2035 Plan can support their existing work and inspire new cross-sector efforts to improve cardiovascular health and equity in their communities.

Webinar: The Lifestyle Change Program: Evidence-Based Support for Improved Patient Health

Date: March 24, 12-1pm

This webinar is an opportunity for providers, care team members, and clinic leaders to learn more about the Lifestyle Change Program. This essential tool is an evidenced-based lifestyle change program aimed at helping participants make lasting changes like increasing physical activity, making healthy food choices, managing stress, and more! The program is covered by most health plans, including Medicare, and can be delivered to groups in-person or online by health care systems and community-based organizations. Learn how MDH staff can support you as you explore offering the Lifestyle Change Program or referring your patients to an existing program.
