One Week Left to Submit a Proposal for Health Care Homes Learning Days!
There is one week remaining to submit a presenter proposal for the upcoming Health Care Homes Learning Days event.
Health Care Homes Learning Days brings together providers, care coordinators, administrators, and partners from across the state to share insights, lessons learned, and effective practices that support patient-centered, community-based care.
We have received several strong proposals to date and welcome additional submissions that highlight innovative programs, practical tools, quality improvement efforts, and real-world experiences within the Health Care Homes model.
The deadline to submit a proposal is 5:00 p.m. on February 13.

This webinar is an opportunity for providers, care team members, and clinic leaders to learn more about the Lifestyle Change Program. This essential tool is an evidenced-based lifestyle change program aimed at helping participants make lasting changes like increasing physical activity, making healthy food choices, managing stress, and more! The program is covered by most health plans, including Medicare, and can be delivered to groups in-person or online by health care systems and community-based organizations. Learn how MDH staff can support you as you explore offering the Lifestyle Change Program or referring your patients to an existing program.
