Deadline Extended: Health Care Homes Learning Days Presenter Proposals

We’ve received several strong presenter submissions for the upcoming Health Care Homes Learning Days event and appreciate the thoughtful proposals shared so far.

To allow additional time for interested presenters, the submission deadline has been extended to 5:00 p.m. on February 13.

This extension provides an opportunity for additional individuals and organizations to submit proposals highlighting innovative approaches, practical strategies, and meaningful experiences that support the Health Care Homes model and community-based care.

If you are considering submitting a proposal—or would like to share this opportunity with colleagues—we encourage you to take advantage of the extended deadline.

Webinar: Minnesota’s Action Plan to Address Cardiovascular Disease, Stroke, and Diabetes 2035

Date: Feb 26, 12-1pm

Join us for an engaging overview of the Minnesota 2035 Plan (MN 2035 Plan) – a statewide, community-driven roadmap to prevent and manage cardiovascular disease, stroke, and diabetes with a strong focus on health equity. This webinar will highlight how the Plan aligns with the work of Health Care Homes and why their role is essential to achieving its vision.

Participants will explore the Plan’s key goals and priority outcomes, learn how it was shaped through community engagement, and identify practical opportunities for partnership and collaboration. Attendees will leave with a clearer understanding of how the MN 2035 Plan can support their existing work and inspire new cross-sector efforts to improve cardiovascular health and equity in their communities.
