Rural Health Transformation Program Update

The Minnesota Department of Health has been awarded more than $193 million through the Rural Health Transformation Program for rural health care across our state. The funds will be released on an annual basis from Centers for Medicare & Medicaid Services (CMS). The Office of Rural Health and Primary Care is working to implement the first-year funding distribution with valuable input from partners and stakeholders. 

Over the next few months, funding opportunities will be released to take action on Minnesota's strategic initiatives:

  • Community-based preventive care and chronic disease management.
  • Recruiting and retaining talent in rural communities.
  • Sustaining access to services to keep care closer to home.
  • Creating regional care models to improve whole-person health.
  • Investing in technology, infrastructure, and collaboration for financial viability.

Rural Health Advisory Committee Vacancy

The Rural Health Advisory Committee is seeking an Allied Dental Member. The Rural Health Advisory Committee (RHAC) is a statewide forum for rural health interests. The 22-member committee includes licensed health care and higher education professionals, legislative officials and consumers. RHAC advises the Commissioner of Health and leaders in other state agencies on rural health issues. Visit Open Positions Search to apply.

Who provides telehealth services?

Health care providers in Minnesota provide telehealth services. The Minnesota Department of Health workforce analysis team collects data on the provision of these telehealth services through our health care workforce survey. This data allows us to take a deeper look into how much and which provider types continue to provide telehealth services.

Top ten licensed provider types that use telehealth services

Data collected from the 2024 Health care Workforce Survey revealed that licensed behavioral health practitioners account for 6 out of the top 10 providers that offer telehealth services. Most health care providers use telehealth services for about 25% or less of their care. Telehealth use did not significantly differ across provider age groups. Clinic settings use telehealth most often compared to other health care environments.

Providers that work in urban areas are slightly more likely to offer telehealth services when compared to more rural areas (41% vs. 32%).

Grants and Funding

Pediatric Primary Care Mental Health Training Grant

The Pediatric Primary Care Mental Health Training Grant Program awards grants for the development of new or expansion of existing pediatric mental health training programs located in outpatient primary care clinics. The training programs are designed to increase confidence, skills, and awareness of mental health resources among pediatric primary care providers caring for the mental health needs of pediatric patients.

All applications must be submitted through the ORHPC Online Grants Portal by 4:30 p.m. on Friday Jan. 16.

Summer Health Care Internship Program

The Summer Health Care Internship Program (SHCIP) brings was created to expose interested secondary and post-secondary pupils to careers within the health care professions

All applications must be submitted through the ORHPC Online Grants Portal by 4:30 p.m. on Wednesday Feb. 4.

International Medical Graduates (IMG) Clinical Preparation Grant

International Medical Graduates (IMG) Clinical Preparation Grant supports clinical preparation for Minnesota immigrant international medical graduates (IMGs). This grant program addresses barriers to practice and facilitates pathways to assist immigrant IMGs to integrate into the Minnesota health care workforce, specifically by preparing IMGs for admission into medical residency programs. 

All applications must be submitted through the ORHPC Online Grants Portal by 4:30 p.m. on Friday Feb. 13.

Call for Presenters: Health Care Homes Learning Days

Do you have practical experience, innovative approaches, or lessons learned that could benefit other clinics and care teams? The Health Care Homes program is now accepting presenter proposals for its upcoming Health Care Homes Learning Days event, taking place May 13, at the Heritage Center of Brooklyn Center.

Health Care Homes Learning Days is a professional development event designed for clinics and health care partners to share knowledge, learn from peers, and explore strategies that support patient-centered care. Sessions focus on real-world applications, improvement work, and ideas that clinics can take back and apply in their own settings.

We are seeking presenters from clinics, hospitals, and partner organizations who are interested in sharing:

  • Best practices or improvement work from Health Care Homes participation
  • Lessons learned from care coordination, quality improvement, or team-based care
  • Innovative tools, workflows, or approaches used in clinical settings
  • Strategies that support patient engagement, equity, or care delivery

Presenters do not need to be professional speakers—practical experience and a willingness to share what works (and what doesn’t) are highly valued.

Resources Available from MDH’s Long COVID Program

Long COVID and other post-viral infection associated chronic conditions can significantly impact daily life and are a leading causes of new disabilities. These tools help address gaps in care for this population by providing practical, up-to-date guidance on diagnosing, managing, and supporting patients experiencing Long COVID. 

In conjunction with Stratis Health, MDH has released four evidence-informed tools to support providers in caring for patients with Long COVID.


Stakeholder Perspectives on Rural Hospital Closures and Prevention Survey

Researchers from the University of Illinois Chicago invite participation in a survey. Led by Dr. Kiruthika Balakrishnan, “Developing a Machine Learning–Based Early Warning System to Prevent Rural Hospital Closures: Integrating Predictive Analytics with Stakeholder Perspectives” seeks to combine predictive data modeling with input from rural health stakeholders to better understand—and ultimately prevent—rural hospital closures.
