Call for Presentation Proposals Now Open!
The Health Care Homes Program is now accepting presentation proposal submissions for Health Care Homes Learning Days 2026, taking place on Wednesday, May 13, 2026, at the Heritage Center of Brooklyn Center.
Health Care Homes Learning Days is a statewide conference that brings together clinic teams, partners, and subject matter experts to share practical, real-world strategies that strengthen patient and family engagement, enhance care coordination, support integrated care teams, and advance health equity across Minnesota’s healthcare system.
We invite individuals and organizations to submit proposals for one-hour breakout sessions that showcase effective approaches, tools, partnerships, and lessons learned that can be applied directly in practice. Sessions may highlight innovations, successful workflows, community collaborations, or implementation experiences that support the Health Care Homes model.
The proposal submission deadline is Jan 30.

The Minnesota Department of Health contracted with St. Paul-based CHW Solutions to develop tools to assist Minnesota Community Health Worker service providers in gaining understanding of billing guidelines for Medicaid and Medicare for health education and Community Health Integration (CHI) services.
This webinar will provide an overview of the tools and how organizations can use them to deliver billable CHW services. Webinar content will be directed to health care organizations, FQHCs, tribal organizations, community based organizations, community hubs and local public health departments. Anyone can be a vaccine advocate! This training aims to empower participants to become more comfortable having conversations about vaccines. We will cover how to talk about vaccine basics, vaccine decision making, and navigating difficult mis- and disinformation. Participants will learn strategies for addressing vaccine misinformation, navigating sensitive conversations with empathy, and promoting the importance of vaccines confidently and respectfully. This training is ideal for anyone who works with parents or families and could encounter a conversation about vaccines.
