Save the date for Learning Days

 

Call for Presentation Proposals Now Open!

The Health Care Homes Program is now accepting presentation proposal submissions for the Learning Days 2026, taking place on Wednesday, May 13, 2026, at the Heritage Center of Brooklyn Center.

Health Care Homes Learning Days is a statewide conference focused on sharing practical strategies that support patient and family engagement, care coordination, integrated care teams, and health equity across Minnesota’s healthcare system.

We invite individuals and organizations to submit proposals for one hour breakout sessions that highlight effective approaches, tools, partnerships, and lessons learned that can be applied in real world practice.

The proposal submission deadline is January 30, 2026.

Additional details, including submission guidelines and the proposal form, are available on the Health Care Homes Learning Days website.


Reminder: Learning Resources Exclusive to Certified Clinics in 2026

In case you missed our earlier announcement, beginning January 2026, Health Care Homes learning resources will be available exclusively to Health Care Homes certified clinics.

This shift allows the Health Care Homes Program to focus more intentionally on topics that are directly relevant to certified clinics and Health Care Home priorities. Our growing learning library represents a meaningful benefit for clinics invested in the Health Care Homes model, offering free e-learning opportunities and continuing education credits that support ongoing development and team engagement.

Additional information and guidance will be shared after the beginning of the year as these changes take effect. If you have questions in the meantime, please contact your Performance Improvement Nurse Specialist or email health.healthcarehomes@state.mn.us.


Webinar: Patients as Partners: A New Model for Care Delivery Innovation

Date: Thursday, Jan. 8, 2026 noon-1p.m. (New date – previously scheduled for December 4)

We know improving healthcare delivery is most effective when patients and families co-design solutions. This webinar explores how Gillette Children’s integrates lived experience into systems change, using patient-family partnerships to enhance care delivery outcomes. Attendees will learn strategies to co-create meaningful improvements from concept to dissemination. Through a five-year history of Family Engagement at Gillette and real-world examples, participants will discover how to foster authentic engagement without tokenism. Ideal for clinicians, administrators, and patient/family advocates, this session offers practical, evidence-informed approaches to advancing patient-centered care delivery through genuine collaboration.

Webinar: Explore New Tools for Understanding Billable Community Health Worker Services

Date: Wednesday, Jan. 21, 2026 noon-1pm (New date - previously scheduled for December 18)

The Minnesota Department of Health contracted with St. Paul-based CHW Solutions to develop tools to assist Minnesota Community Health Worker service providers in gaining understanding of billing guidelines for Medicaid and Medicare for health education and Community Health Integration (CHI) services.

This webinar will provide an overview of the tools and how organizations can use them to deliver billable CHW services. Webinar content will be directed to health care organizations, FQHCs, tribal organizations, community based organizations, community hubs and local public health departments. Information about how to access additional resources and technical assistance will also be shared. The tools are now available on the MDH website: Community Health Worker Resources for Employers

Webinar: Becoming a Vaccine Advocate

Date: Tuesday January 27, 2026 12-1pm

Anyone can be a vaccine advocate! This training aims to empower participants to become more comfortable having conversations about vaccines. We will cover how to talk about vaccine basics, vaccine decision making, and navigating difficult mis- and disinformation. Participants will learn strategies for addressing vaccine misinformation, navigating sensitive conversations with empathy, and promoting the importance of vaccines confidently and respectfully. This training is ideal for anyone who works with parents or families and could encounter a conversation about vaccines.
