As part of your site’s enrollment in the UUAV program, you are required to complete forms for our annual re-enrollment process. Your ordering privileges may be suspended if we do not receive the completed forms by the deadline.
Submit the following forms by Nov. 30:
- UUAV Provider Agreement: This must be completed and signed by each site’s medical director every two years.
- The annual report of the number of immunized adult patients.

- If your site redistributes vaccine to satellite sites, you must submit one annual report that includes the data for those satellite sites.

- If your site has a system-level contact, they may be responsible for submitting this report for all sites in your system. Connect with the appropriate contacts in your organization to confirm who is completing this requirement.
- Annual contact and address verification form: This form is used to verify and update our contact information and addresses for all sites.
Replacement method sites will also be emailed a link to complete the Replacement Method of Vaccine Management Agreement.
You will be required to complete additional forms if you are also enrolled in MnVFC. The email we send your site will contain links to all the required forms for your site, including both UUAV and MnVFC forms if applicable.

Starting on Dec. 1, McKesson is offering a FluMist replacement program for product purchased through the Centers for Disease Control and Prevention (CDC) contract for the 2025-26 season. This applies to both MnVFC and UUAV FluMist doses. The program allows for the replacement of unused, expiring FluMist doses at no cost to help you maximize product usage opportunities. Doses will be replaced in increments of 10.
- Doses must have been purchased on the CDC contract. Private doses are not eligible.
- Product expiration date must be on or before Feb. 1, 2026.
- Requests for replacement of expiring doses may be made no more than 15 days prior to the expiration date stamped on the sprayer.
- AstraZeneca will replace expired doses of FluMist while supplies last.
