Introducing Health Care Homes’ New Tagline
Minnesota’s Health Care Homes program is excited to announce that we are adopting a new tagline, “Primary Care Clinics That Put You First.”
The new tagline directly addresses a very common misconception. Too many people confuse Health Care Homes with traditional home health care services or nursing home facilities. We want to clarify that Health Care Homes is a clinic-based model for coordinated healthcare. Health Care Homes are primary care clinics that have met our state’s patient-centered requirements for certification.
The new tagline specifically calls out our clinic-based model and emphasizes our patient-centered approach. We are enthusiastic that this shift in our tagline will help people more clearly understand what Health Care Homes are and the valuable work that Health Care Homes clinics do all across our state.
Enhancing Value for Certified Clinics: Learning Resources Exclusive in 2026
As of January 2026, Health Care Homes learning resources will only be available to certified clinics. This allows us to focus on topics that are specific to and prioritized by Health Care Home clinics. Our learning library has grown in both quality and impact and represents a meaningful benefit to clinics who are vested in the Health Care Homes model. Certified clinics can leverage these resources to support their ongoing development, strengthen engagement, and experience the added value of certification through free e-learning opportunities and continuing education credits.
The HCH certification model supports higher levels of achievement for your provider teams and the patients they serve, while giving you access to free, high-quality learning resources.

This webinar describes translation and implementation of an evidence-based pediatric to adult healthcare transition (HCT) program by a team of Gillette Children’s clinicians, lived experience partners, and operational specialists. Translated HCT tools (e.g., transition readiness assessment, HCT EMR order set for providers, clinician/youth HCT resources) and processes (e.g., HCT policy, clinician/youth education) grounded in patient and family-centered care and The Six Core Elements of HCT™ are shared, along with process and outcome measures to evaluate adoption and sustainability. Intended for any clinicians or community members interested in building a sustainable HCT program that meets HCH certification requirements.

The Minnesota Department of Health contracted with St. Paul-based CHW Solutions to develop tools to assist Minnesota Community Health Worker service providers in gaining understanding and accessing resources to bill Medicaid and Medicare for health education and Community Health Integration (CHI) services.
This webinar will provide an overview of the tools and how organizations can use them to access sustainable funds for CHW services through medical billing. Webinar content will be directed to health care organizations, FQHCs, tribal organizations, community based organizations, community hubs and local public health departments. Information about how to access additional resources and technical assistance will also be shared.
