Patients living with opioid use disorder (OUD) often face persistent obstacles to getting the care they need. Health Care Homes (HCHs) and care coordination teams are in a unique position to make a real difference, especially when it comes to connecting patients with effective treatment options like Medications for Opioid Use Disorder (MOUD).
