The application for the Uninsured and underinsured Adult Vaccine (UUAV) program is now open. Completing the application helps us manage limited vaccine funding and ensure access for un- and underinsured adults in Minnesota. All current participants must complete this application to remain enrolled in the program.
Application details:
- Enrollment period: Oct. 1, 2025 - Sept. 30, 2027.
- Application deadline: July 31, 2025 (end of day).
- Notification of enrollment decisions: By Sept. 1, 2025.
When you begin the survey, you can save an incomplete response and return to it later. Systems applying for multiple sites should submit one application and list all participating sites within it.
All enrolled sites should have received an email with a unique link to complete the application. If you did not receive your unique application link, contact us at health.uuadultvax@state.mn.us for assistance.
