Resource webpage for condition follow-up
Last month, we launched a new LPH Partner Resources webpage to make it easier for local public health staff to find materials for CYSHN programs—including Birth Defects, EHDI, Heritable Conditions, and congenital CMV.
You do not need a login to view or download materials. We created it in response to delays getting new staff into SharePoint– this way, you can access what you need without the wait.
Some materials, like videos and Follow Along Program resources, are still only available on our SharePoint site. We will continue maintaining both spaces and providing SharePoint access for new and existing staff.
Reminder: timely follow up on CYSHN cases in MEDSS
As a reminder, please attempt to follow up with families within one month of the “Date Event Sent to LPH” in MEDSS. Local public health contacts have the rest of the quarter that the event was sent, plus one additional quarter, to complete follow-up documentation and submit it for reimbursement.
Thank you for all your efforts to reach out to families referred through MEDSS in a timely manner!
Program updates
Early hearing detection and intervention
New resources for families: available in 4 languages

A video describing early intervention and its importance for children who are deaf and hard of hearing is now available in English/ASL, Hmong, Somali, and Spanish. Bookmark the Supporting Deaf & Hard of Hearing Children Early playlist to easily access all four videos.

The EHDI “resources for families” binder is now available electronically in English, Hmong, Somali, and Spanish. All four versions can be accessed on the Resources for Families webpage under the heading “What do I need to know if my child is identified with hearing loss?”
Please help us in sharing these resources with colleagues and the families you serve.
Follow Along Program
Semi-annual data reporting - due July 31
The Follow Along Program collects two types of reports from all local public health agencies implementing the program:

Semi-annual (6-month) report: January 1 – June 30, 2025

Semi-annual (6-month) report: July 1 – December 31, 2025
The shortened semi-annual data reporting form contains a new question about ‘active’ vs. ‘new’ clients. Below is some guidance on how this can be calculated:

Active clients are the total number of clients during the reporting period (continuing clients + new clients). Continuing clients are those who are not new during the reporting period but are still actively completing ASQs or being connected to resources. New clients are those who were enrolled during the reporting period. If the reporting period is January – June 2025 we look at clients within and outside this range.

Active Clients = New Clients (Enrolled within Jan – June 2025) + Continuing Clients (from before Jan 2025)
Please complete this process no later than July 31.
Follow Along Program invoicing - due Aug. 20
We encourage staff to invoice MDH every six months (e.g. half the annual award) in February and August, following the semi-annual data submission deadlines in January and July. Invoices reflect an amount for the general implementation of the program; community health boards are not expected to submit a budget by category of expense.
After completing bi-annual data reporting for the program, please submit your invoice using the data reporting survey, the separate invoice survey, or send your invoice by email to health.cyshn@state.mn.us no later than August 20.
July 28 – Proof Alliance “Caring for Transition Age and Adults” webinar
 This caregiver support group on Monday July 28 at 8 p.m. is facilitated by caregiver Melissa F. and is specially designed for caregivers of teens and adults with fetal alcohol spectrum disorders (FASD) who are navigating the transition to adulthood. It’s a place to connect with others who understand the unique challenges and joys of supporting young adults with an FASD. Please register to attend.
July 29 – Follow Along Program data system open office hour
CYSHN is working toward transitioning local public health agencies and implementation staff from the current Follow Along Program software to MEDSS. We will be holding an office hour on July 29 from 9 – 10 a.m. to answer questions about this transition. Please keep an eye out for the calendar invite. You are welcome to forward the invite to any staff involved in FAP implementation that may wish to ask questions about the data system transition.
We are very grateful for the feedback you have shared throughout the Follow Along Program redesign process. You are welcome to submit office hour questions via email to health.cyshn@state.mn.us.
Aug. 12 – Ages and stages training
The next Introduction to ages and stages questionnaires (ASQ) training will be held Tuesday, Aug. 12, from 8:30 - 11:30 a.m. via Microsoft Teams. This training is intended for family home visitors who want an introduction to understanding the use and purpose of these developmental and social emotional screening tools. The training is also open to local public health staff implementing the Follow Along Program. Please visit the family home visiting training and professional development webpage under ‘current trainings’ more information and to register.
Sept. 4 – CYSHN condition follow-up virtual community of practice
The next community of practice meeting for CYSHN condition follow-up program contacts is Thursday, Sept. 4 from 9 - 10:30 a.m. The purpose of these meetings is to provide routine opportunities for local public health nurses who provide follow-up for children referred by CYSHN programs (Birth Defects, EHDI, Heritable Conditions, congenital CMV) to learn condition-specific information and discuss nursing practice. The meeting will include a presentation from staff at the Department of Human Services about Medical Assistance under the TEFRA option and the State Medical Review Team. We will be offering one contact hour of continuing education for attending.
All current CYSHN condition follow-up LPH nurses are welcome to attend. Please register for this virtual, optional meeting. If you have any questions you would like to submit in advance, please email them to dana.janowiak@state.mn.us.
Sept. 16 and Sept. 23 – Charting the LifeCourse Good Life Groups hosted by Family Voices
Family Voices of Minnesota is offering a series of Charting the LifeCourse Good Life Groups for Minnesota parents raising children with disabilities. Good Life Groups are collaborative learning groups where families use the Charting the LifeCourse framework to discuss, plan, and work towards achieving their vision of a fulfilling life for their family member with a disability. Families should plan to attend both sessions. Please register to attend.

Sept. 27 – Minneapolis ACHA Walk for 1 in 100
On Saturday, Sept. 27 at 9 a.m. at Hyland Hills Park in Minneapolis, the Adult Congenital Heart Association is hosting its annual walk for people living with congenital heart disease.
More than 2.5 million people live with congenital heart disease across the country. Since 1998, this organization has been empowering and advocating for the 1 in 100 children and adults impacted by this disease.
