Health Care Homes Learning Days 2025 Strengthens Minnesota's Health Care Community

The Minnesota Department of Health's Health Care Homes program successfully hosted Learning Days 2025 on May 14 at the Heritage Center of Brooklyn Center, bringing together more than 170 health care professionals from across the state.

Advancing Care Innovation

Providers, clinic managers, community partners, and public health professionals participated in sessions on behavioral health integration, health equity strategies, adolescent care transitions, and innovative care delivery models. Dr. David Satin's keynote presentation and workshop provided practical approaches to health care transformation that participants are implementing in their practices.

Strengthening Partnerships

Learning Days facilitated valuable connections between primary care clinics, health care systems, community organizations, and public health agencies. These partnerships are essential for addressing health disparities and improving care coordination across Minnesota's communities.

Building on Success

The positive response from attendees reinforces the value of bringing together Minnesota's health care workforce for shared learning. The Health Care Homes program is incorporating feedback into planning for Learning Days 2026.

This annual conference continues to advance the Health Care Homes model across Minnesota, supporting accessible, high-quality health care for all residents.

Webinar - Building Trust in Culturally Diverse Clinic Environments

Date: June 5; noon - 1pm

Welcoming clinic environments and trusted relationships between providers and patients contribute to building health equity in our state. In this session, participants will learn about the cultural beliefs of Hmong, Somali, and Spanish language immigrant and refugee communities in Minnesota, with a focus on cultural perspectives and practices surrounding illness and healing. Participants will learn about common causes of mistrust toward medical institutions and leave this session with a better understanding of these immigrant communities in Minnesota and strategies to build trusting relationships with patients.
