Learning Days 2025 - Call for Proposals - Deadline This Friday

The Health Care Homes program is extending the timeline for presentation proposals for our annual Learning Days conference on May 14 at the Heritage Center of Brooklyn Center. Your expertise and experiences are valuable to Minnesota's healthcare community. Whether you're from a primary care clinic, behavioral health setting, public health organization, or community partner, we welcome your insights and best practices. 

Final proposal deadline: Friday, February 21

Webinar - Improving Health Outcomes
for People with Prediabetes and Diabetes: Evidence, Challenges, Opportunities, and Resources

Date: March 27; noon - 1pm

Those at risk for or living with diabetes often face many challenges in preventing and managing their health conditions. Learn how unique barriers to health such as reduced access to care, unemployment, low incomes, housing cost burdens, or lack of health insurance can make managing chronic conditions such as prediabetes and diabetes more difficult. We will also discuss ways to address those barriers.

Presenter:

Lorena Drago, MS, RDN, CDN, CDCES,  is a registered dietitian, speaker, author, consultant, and certified diabetes care and educator specialist. She specializes in the multicultural aspects of diabetes self-management education and is an expert in developing culturally and ethnically oriented nutrition and diabetes education materials.
