Learning Days 2025 - Call for Proposals - Deadline Extended
The Health Care Homes program is extending the timeline for presentation proposals for our annual Learning Days conference on May 14 at the Heritage Center of Brooklyn Center. Your expertise and experiences are valuable to Minnesota's healthcare community. Whether you're from a primary care clinic, behavioral health setting, public health organization, or community partner, we welcome your insights and best practices. We're pleased with the response so far but would love more submissions.
