Spotlight: St. Louis County Public Health wins national award for Heart-to-Heart
The Heart-to-Heart program in Duluth recently won a Model Practices award from the National Association of County and City Health Officials. This award recognizes outstanding local public health approaches and shares promising practices with a broader audience. Heart-to-Heart aims to improve birth outcomes related to hypertensive disorders in pregnancy including preeclampsia. Now in grant year two, the program will incorporate a community health worker into the staffing model to support health system navigation and referrals to community resources. Heart-to-Heart is funded by a Promising Practices grant from the Minnesota Department of Health (MDH).

Strong Foundations Mid-year Report available soon!
The 2024 Strong Foundations Mid-year Grantee Feedback Report will be sent later this week! The report includes data on advancing health equity, prenatal recruitment, target caseload, data quality, staff professional development and trainings, and staff recruitment and retention. Conference opportunity: Partners in public health
The Local Public Health Association and MDH are again offering an opportunity for learning and connection: . This conference will be held Nov. 13-15 in Breezy Point and will explore new approaches for transforming Minnesota’s public health system.

Infant Mortality Awareness Week is next week!
Mark your calendar and get the word out! Sept. 15-21 is Infant Mortality Awareness Week in Minnesota. MDH will post educational messages related to infant mortality on social media throughout the week. Contact the infant and child health unit with questions at health.infantmortality@state.mn.us.
Maternal and Child Health Task Force – family home visiting subcommittee members selected
The Maternal and Child Health (MCH) Task Force, along with MDH family home visiting leadership, selected six additional members to be a part of the MCH Task Force family home visiting subcommittee. The purpose of the family home visiting subcommittee will be to use a professional home visiting and community-based lens to advise the MDH family home visiting program. This subcommittee will serve as an independent, impartial sounding board, and will assist staff in determining important activities and priorities related to family home visiting. The group may also assist in evaluating the overall performance of MDH family home visiting.
There was a vast amount of interest across the state for the additional spots for the external advisory group. Additional members were selected on the following minimum criteria:
- Is currently working in a home visiting role (as a home visitor, manager, or supervisor).
- Is enrolled in or has graduated from a home visiting program.
- Is a family member of or is close to someone who is enrolled in or graduated from a home visiting program.
- Is a community partner of a home visiting program.
Below are the names of the individuals selected to be a part of the MCH Task Force subcommittee.
