This webinar will provide more detailed information on the billable Community Health Worker (CHW) services in Minnesota as a result of the new Medicare Physician Fee Schedule Billing Codes for Community Health Integration and Principal Illness Navigation. The webinar will also have a Q&A segment and will include representatives from both Minnesota Department of Health and Department of Human Services.
