Nearly 150 primary care professionals from throughout the state took time out to upgrade their skills and network with peers at this annual day of learning for Health Care Homes stakeholders and their partner organizations.

Webinar: Understanding Billing for CHW Services in MN, June 5, Noon – 1:00 PM

Join us for an informative one-hour webinar on Understanding Billing for Community Health Worker (CHW) Services in Minnesota. The webinar will provide an overview of the new Medicare Physician Fee Schedule Billing Codes for Community Health Integration and Principal Illness navigation, highlight an example of successful billing and reimbursement for these new codes, and discuss the potential opportunities for these codes in MN. The webinar will also provide an overview of Minnesota Health Care Program codes for CHW Education services and highlight examples of successful strategies for billing, reimbursement, and sustainability.

Webinar: Trends in Mental Health & Levels of Care*, July 16, Noon – 1:00 PM

Our mental health care systems need a transformation. We need to normalize conversations around mental health, invest in education, identification, and prevention, create better ways to access care and be better prepared for crisis. Reviewing current trends in mental health in youth and different levels of care as potential access points will provide attendees with a better understanding to offer comprehensive care. Multiple resources will be reviewed as well as access to free psychiatric consultations for prescribing providers. Presented by Joshua Stein, MD Board Certified Child & Adolescent Psychiatrist PrairieCare/Psychiatric Assistance Line (PAL).

Peer-to-Peer Networking Paused for 2024

Health Care Homes stakeholder-driven peer to peer networking initiative is taking a break for the rest of 2024. It will be re-evaluated as part of the 2025 learning plan. Thanks to the Health Care Homes stakeholders who designed this program and to all who participated in it.
