Learn how to address social determinants of health that play a pivotal role in patient outcomes. Discover how the connection model and other resources, including the Take 5 model, can help you identify gaps and opportunities in practice and partnership to roll out your own plan of intervention.

Join us online in December for a webinar on how Health Care Home clinics are responding to primary care workforce shortages post-COVID.
