This webinar is a continuation of the September 26 webinar, Overcoming Socioeconomic Barriers in Rural Minnesota Using a Connection Model. In this session, Dani Protivinsky, CentraCare, will further explore where and how to start building your own unique partnerships at the clinic and community level. Learn how to address social determinants of health and play a pivotal role in patient outcomes. Discover how the connection model and other resources, including the Take 5 model, can help you identify gaps and opportunities in practice and partnership to roll out your own plan of intervention.
