Annual Immunization Status Report and Infectious Diseases in Minnesota Schools

Please make sure the school nurse for your institution receives the message below.

This is an annual communication about vaccine-preventable diseases (VPDs) from the Minnesota Department of Health (MDH). The School Health Personnel Immunization and Disease Reporting website is organized to provide you with the information you need to implement Minnesota school immunization law, report diseases, and to support you in providing information to students and families. Also included is a link to directions for completing the Annual Immunization Status Report (AISR).

Schools need to:

Complete the AISR by December 1, 2023.

Visit the School Health Personnel Immunization and Disease Reporting website to complete the 2023 AISR.

Report diseases to MDH.

Information about VPDs and details on how and when to report VPDs such as pertussis, influenza, or varicella can be found at School Health Personnel Immunization and Disease Reporting under Report disease. MDH also has tools, templates, and staff available to help with responding to and notifying staff and families when needed.
To report cases of mumps, pertussis, varicella (chickenpox) or zoster (shingles) use the Vaccine Preventable Disease (VPD) Reporting Form.
To report outbreaks of influenza, COVID-19, and gastrointestinal illness outbreaks such as norovirus please use the Respiratory Illness and Gastrointestinal Illness Outbreak Reporting Form.

Bookmark the School Health Personnel Immunization and Disease Reporting website.
Post the School Reporting of Vaccine-Preventable Diseases table where it is visible.
Subscribe to School Immunization and Vaccine-Preventable Disease Information to receive email updates for schools on immunizations and VPDs. Select the red envelope at the top right side of the page.
Consider including VPD fact sheets in mailings to parents or post links to them on your school website.
Use MIIC (Minnesota Immunization Information Connection) to look up and keep track of students’ immunization histories, immunizations due, and print immunization records.
