Register today for a free one-hour virtual training designed to enhance intercultural effectiveness for Health Care Homes stakeholders and community partners. Presented by HealthFinders Collaborative, a southern Minnesota community health center and health care home, this interactive webinar will provide valuable insights and tips for building strong connections with culturally unique patient populations, with a focus on serving the Latinx and Somali communities.

In addition, attendees will learn how to build allyship and partnership within underserved communities, moving from personal connections to a powerful collective impact. Don't miss out on this exciting opportunity to expand your cultural competency and make a positive change in your community.

New E-Learning: Updated Foundations of Health Care Homes Certification 

An updated version of the Foundations of Health Care Homes Certification e-learning course is now available on the MDH Learning Center. The course covers the Health Care Homes progression model and the standards and requirements for certification and recertification as a Health Care Home. The course is designed for primary care providers considering Health Care Home certification, as a resource for clinical staff in the certification or recertification process, and as an orientation for care team members to learn about the Health Care Home model.  

The nine-chapter course is free, accessible on demand, and worth 3.5 credit hours for those who complete the entire course. To access the course, log in to the MDH Learning Center and search for Foundations of Health Care Homes Certification. Additional information and resources are available on the Health Care Homes website.

Learning Days 2023 – So Good to See You!

More than 200 Health Care Homes clinics and community partners showed up for Learning Days 2023, the first “in person” event since 2019! It was so good to see everyone again, and we had a great day of learning and networking. Thanks to everyone – our planning team, presenters, sponsors, exhibitors, and participants. We hope to see you all next year!
