Learning Days 2023

Registration is now open for Learning Days 2023, returning live and in person on May 18, 2023, from 8:30 am – 4:00 pm at River’s Edge Convention Center in St. Cloud, MN. The day will feature two keynotes, nine breakout sessions, and a special networking workshop led by Health Care Homes Practice Improvement and Integration Specialists.

Register Now for Motivational Interviewing Skills Refresher

HCH E-Learning

Sign up now for this free training that will focus on the essential principles and techniques of Motivational Interviewing, an Evidence-Based Practice designed to help facilitate and support patient change. This one-day refresher will be highly experiential, giving practitioners hands-on exposure and practice. Enrollment is limited to certified Health Care Home clinics and is capped at 30 per workshop; no more than two enrollees per organization, please. 7 credit hours.
