Greetings Colleagues!
Thank you to the many facilities and staff who have completed the COVID-19 Response survey! This is a friendly reminder to those who have not had an opportunity to complete the survey that it is still open until end of day Friday, February 24, 2023.
The purpose of this survey is to inform MDH of challenges unique to correctional facilities for future infectious disease response planning.

The survey should take no longer than 10 minutes to complete. You will not be asked to provide your name or facility name; however, we request demographic information on facility type, size, and region to better understand where resources may be needed.
Your responses will be combined with those of other participants and will be shared with state agency staff responsible for supporting the COVID-19 response in corrections facilities. Results of the survey will be shared with correctional facilities at a future webinar.
