Greetings Colleagues!
The Minnesota Department of Health (MDH) COVID-19 Highly Impacted Settings Unit is sending this survey to be used for ongoing COVID-19 response planning in correctional facilities. The purpose of this survey is to inform MDH of challenges unique to correctional facilities for future infectious disease response planning.

Please complete the survey by February 24, 2023. The survey should take no longer than 10 minutes to complete. You will not be asked to provide your name or facility name; however, we request demographic information on facility type, size, and region to better understand where resources may be needed.
Your responses will be combined with those of other participants and will be shared with state agency staff responsible for supporting the COVID-19 response in corrections facilities.
