The Foundations of Health Care Homes e-learning course is being updated to reflect upcoming changes in Health Care Homes certification levels and processes. The first version of the Foundations course is no longer available. If you took this course, it will remain on your MDH Learning Center transcript. Don’t forget that Learning Days 2022 content and resources are still available to registered participants on the Learning Days website through the end of 2022. Certificates of attendance will be e-mailed within two weeks of course completion. If you were unable to attend Learning Days live, you can still register and pay to access recorded content and receive certificates of attendance.
