Annual Immunization Status Report and Infectious Diseases in Minnesota Schools

Please make sure the school nurse for your institution receives the message below.

This is an annual communication about vaccine-preventable diseases (VPDs) from the Minnesota Department of Health (MDH). The School Health Personnel Immunization and Disease Reporting website is organized to provide you with the information you need to implement Minnesota school immunization law, report diseases, and provide information to students and families. Also included is a link to directions for completing the Annual Immunization Status Report (AISR).

We need you to:

Complete the AISR by December 1, 2022.

The 2022 AISR application is being updated. Make sure to subscribe to the School Health Personnel Immunization and Disease Reporting website and we’ll send you an email when it is ready.

Report diseases to MDH.

• Vaccine preventable diseases and details on how and when to report VPDs such as pertussis, influenza, or varicella can be found at School Health Personnel Immunization and Disease Reporting.
• For influenza reporting please utilize the REDCap reporting tool found at the Influenza-like Illness Report Form Minnesota Schools.
• For COVID-19 reporting, review instructions that include how to report found at Schools and Child Care COVID-19 Case Reporting & Best Practice Recommendations.
• Gastrointestinal illness outbreaks, such as norovirus and packet of materials that include how to report and respond to a norovirus outbreak in your school can be found at Foodborne Outbreaks at Schools.

We recommend you:

• Bookmark the School Health Personnel Immunization and Disease Reporting website.
• Subscribe to School Immunization and Vaccine-Preventable Disease Information to receive email updates for schools on immunizations and VPDs. Select the red envelope at the top right side of the page.
• Post the School Reporting of Vaccine-Preventable Diseases table where it is visible.
• Consider including VPD fact sheets in mailings to parents or post links to them on your school website.
• Use MIIC (Minnesota Immunization Information Connection) to look up and keep track of students’ immunization histories, immunizations due, and print immunization records.
