This course provides an overview of the Juniper network of Minnesota community-based resources for individuals with chronic diseases. The course is designed for providers who are interested in referring their patients to Juniper's evidence-based disease prevention and self-management courses. Developed in partnership with the Office of Statewide Health Improvement Initiatives.

If you’re looking for an efficient way to earn CEU credits, don’t forget to visit the MDH Learning Center, where you’ll find a variety of free, on demand, CEU-eligible learning opportunities that will help you enhance your skills and improve your practice. Start by logging into the MDH Learning Center and browse the course catalog where you’ll find close to 100 courses generated by MDH for its stakeholders.
Courses are organized into four folders. You can find Health Care Homes courses in the Health Care Access and Quality Folder. Most courses can be completed quickly when you have time, but if you run out of time to finish, you can return and pick up where you left off.

The HCH Peer to Peer Networking pilot continues this fall with two events in September and November. Dates and topics will be confirmed soon in an upcoming LEARN bulletin and on the Health Care Homes website.

Don’t forget that Learning Days 2022 content and resources are still available to registered participants on the Learning Days website through the end of 2022. Certificates of attendance will be e-mailed within two weeks of course completion. If you were unable to attend Learning Days live, you can still register and pay to access recorded content and receive certificates of attendance.
