Electronic monitoring consent website and updated forms

We are pleased to present a new webpage for easy consumer access to Electronic Monitoring Consent Forms: Electronic Monitoring Consent.

Updated: Use revised versions of consent forms dated 1/20/20.

In response to public comment, we added clarifications on the consent forms for residents and resident representatives related to the 14 day exception. Also, each of consent forms have been corrected for formatting errors.

Facilities must make the consent forms available to residents and inform residents of their option to conduct electronic monitoring. This means that facilities must make the 1/20/20 versions available to residents as soon as possible, but no later than February 1, 2020.
