a full day event focused on behavioral health integration in Minnesota. Joan
King from the National Council of Behavioral Health will be the featured
speaker. The event is free and lunch
will be provided; registration is limited to 150 participants.

Finding Our Way to Integrated Care
The MDH Health Care Home
program invites your participation in a northern Minnesota regional
convening* of certified Health Care Home
clinics, Behavioral Health Home services providers, Certified Community
Behavioral Health Clinics and other providers and community partners interested in primary care, behavioral health integration.
Network and connect with each other and learn skills for collaborative
communication and quality improvement. Facilitated by the Institute for
Clinical Systems Improvement (ICSI), this event is free and lunch will be
provided.
*Enrollment not limited to providers within specific geographical
borders.
