Integrating Primary Care and Behavioral Health
The MDH Health Care Homes program invites your participation in a central Minnesota regional convening* for certified Health Care Home clinics, Behavioral Health Home services providers, Certified Community Behavioral Health Clinics, and partner organizations interested in working together to enhance whole person care. Facilitated by the Institute for Clinical Systems Improvement (ICSI), this event is free and lunch will be provided; registration is limited to 100.

New E-Learning Course Coming in May: MIPS 101
MIPS 101 is a 45 minute
overview of one of the two tracks of the CMS Quality Payment Program and will
help ground you in the basics and prepare you to participate if the program is
new to you. By completing the course, you will be able to:
- Define MACRA, QPP, MIPS, and APMs
- Assess whether you are eligible to participate in the QPP
- Summarize the different MIPS categories
- Explain MIPS scoring and how to use the MIPS Estimator
- Explain how to submit MIPS data
This free online course was

Check Out the MDH Learning Management System!
If you haven’t already done
so, check out learning opportunities available through the new MDH Learning
Management System. HCH has introduced three e-learning courses so
far, including Foundations of Health Care Homes Certification, and Check Up
From the Neck Up (Parts 1 and 2). These
courses are available on demand and are free of charge. Stay tuned for more great learning, coming
soon!

Nearly 350 Health Care Homes
stakeholders from throughout the state met in St. Paul on April 3-4, 2018 to
teach and learn from each other at “Coming Together: Cultivating Health for All.” The annual event features networking and interactive learning
led by health care home clinics, behavioral health providers, and MDH agency
and community partners who are working together to redesign care delivery and
improve health for all Minnesotans.
