Introducing the Winners of the Health Care Homes Innovation Awards

Winners of the first Health Care Homes Innovation Awards were honored on April 3, 2018 at the MDH Health Care Homes annual Learning Days event at the Continuing Education and Conference Center in St. Paul, Minnesota.  The Innovation Awards were created to honor health care home teams or individuals that have achieved outstanding results and improved community health through whole person care, team based care, community partnership, quality improvement and health equity.

Courage Kenny / Allina Advanced Primary Care Clinic, Golden Valley, MN

 

The Courage Kenny Advanced Primary Care Clinic works with a complex patient population that typically experiences not only disabilities, but multiple chronic health conditions and often social factors such as poverty, limited insurance, and poor access to transportation, accessible housing and durable medical equipment.  Their health care home team coordinated care and services and achieved outstanding results, including improved physical and mental health, high patient satisfaction, and significantly reduced client hospitalizations and hospital days.

Hennepin Healthcare Transition Care Team Photo

Hennepin Healthcare Transition Care Team, Minneapolis, MN

 

Using a Lean Management process for continuous quality improvement, the Hennepin Healthcare Transition Care Team addressed issues in care team design and resources, collaboration with other health providers, duplication of services, access to medical records, and billing and reimbursement systems.  Through this process, they integrated the community and transition care areas, increased coordination of care across the continuum and expanded programs and services to enhance the Health Care Home program.

The Community Care Network at Winona Health was recognized because of their innovative partnership with Winona State University students to improve individual health and quality of life.  Over time these partnerships resulted in trusting relationships, improved health and lower costs.
