Welcome to LEARN!

As a new year begins, Health Care Homes (HCH) is pleased to introduce LEARN, a news bulletin to keep you up to date on upcoming HCH learning events, provide tools and practical resources that you can use, and provide links to related learning events.  Help others learn by forwarding or inviting people to subscribe via the "Manage Preferences" link at the bottom of the bulletin or via the HCH website. 

Introducing the New HCH Learning Management System

HCH is excited to introduce a new Learning Management System (LMS) to support your health care home transformation. The LMS is your gateway to online courses, conferences, webinars and other resources designed for health care home stakeholders at all levels of learning.

First e-learning course: Foundations of Health Care Homes Certification

Our first course, Foundations of Health Care Homes Certification, is a series of fourteen modules that takes learners through each of the five Health Care Homes standards and the necessary steps for certification and recertification. Additional training will be added throughout the year. Once you establish a LMS user profile, you’ll be able to access and complete training and create a transcript of your activity.  Ready to get started?

Create a LMS User Profile

The following are step by step instructions to create a user profile in the LMS:

  1. Click here to go to the LMS login page.
  2. Select the Register Now button in the top right part of the screen.
  3. Create a username and password, as well as enter contact information on the page titled "Basic Information". Enter 9501 in the Access Code field. A red star denotes a required field.
  4. Click submit. You will be logged into the LMS.

Register for a course

Register now for our first e-learning course, Foundations of Health Care Homes Certification:

  1. Select Browse Training from the left-hand menu.
  2. Click Minnesota Department of Health to open a list of available courses (Foundations of Health Care Homes Certification.)
  3. Click Register to see a course description and a summary of the fourteen modules comprising the course.
  4. Click the Launch button next to selected module(s) to begin learning.

Learning Days 2018 Registration Opening Soon!

Registration for Learning Days will be coordinated through the LMS, so take time now to establish your user profile.  “Coming Together:  Cultivating Health for All” takes place April 3-4, 2018 at the Continuing Education and Conference Center on the St. Paul Campus of the University of Minnesota.  Watch for your next issue of LEARN for conference information and registration instructions.
