The Minnesota One Health Antibiotic Stewardship Collaborative
is pleased to announce:

The Minnesota Antibiotic Stewardship Honor Roll
for Long-Term Care

Honor roll program objectives:

  • Encourage stewardship commitment from administration, prescribers, and caregiver staff
  • Share long-term care stewardship activities with health professionals and the public
  • Provide incentive for stewardship program improvement and collaboration
  • Publicize the importance of antibiotic responsibility.

Program logistics:

  • Honor roll membership will be annually renewable.
  • Facilities will be eligible for one of three self-selected levels of recognition.
  • Facilities may submit materials at any time, and updates to the honor roll web listing will be made quarterly.
  • The honor roll process will be administered by Minnesota Department of Health.
  • For the first round of recognition, interested long-term care facilities should submit required materials by February 28, 2018.

Levels of recognition:

  • Commitment (Bronze): Demonstrated facility commitment to antibiotic stewardship
  • Action (Silver): Commitment and use of specific interventions to improve antibiotic use
  • Collaboration (Gold): Commitment, use of stewardship interventions, and an engagement in collaborative antibiotic stewardship efforts beyond the long-term care facility
