Effective July 1, 2017 the initial application process for all license types will change. You will need to submit your application, fee and all supporting documentation together. This will help reduce the time it takes to process and issue a license. Applications received without all supporting documents or fees will be returned.
Effective July 1, 2017 licensure fee for a dual license as a speech-language pathologist/audiologist for an initial and renewal will increase to $510.00.
Effective July 1, 2017 late fee for speech-language pathologist, audiologist and dual license as a speech-language pathologist/audiologist will increased to $60.00.
